It is really important that you have your Personal Adviser’s phone number and email address. If you do not have these, please ask for them. At the very least, you should also have the team’s duty number. You may also be able to have contact details for other people in the team if that helps.
- Keeping safe – If you are worried about your safety, make sure you know who to contact and how to get help quickly.
- People that can help you – There are lots of people who can support you as you leave care. This includes the Leaving Care team, Children and Young People’s Services, and people who can help with education, training and work.
- Personal Advisers and Leaving Care Team – Find out how Personal Advisers and Leaving Care Team Leaders support you, stay in touch with you, and help you as you move on from care.
You are not on your own, and support is always available.
